Our approachWe are developing oral Live Biotherapeutic candidates designed to address gut–immune–brain pathways relevant to cognition, neuroinflammation and neurodegeneration.
Our approachOur clinical strategy is biomarker-informed: it is designed to generate human biological evidence first, and to let that evidence guide subsequent indication development.
Clinical strategyIndependent research increasingly connects the gut microbiome with immune, metabolic and central nervous system biology, creating new possibilities for therapeutic intervention in Brain Health.
Explore the pathwaysThe clinical need
Cognitive impairment occurs across several central nervous system disease contexts. In several neurodegenerative conditions, biological changes can precede clinical diagnosis by years. Available treatments remain limited.
Neurodegeneration and neuroinflammation are widely described as multifactorial processes, and no single mechanism accounts for them. Additional therapeutic approaches are needed.

Our science
A growing body of independent research supports a relationship between the gut microbiome and biological processes relevant to Brain Health. NBX Biosciences is developing defined Live Biotherapeutic candidates through a proprietary discovery and development approach. All programs are currently preclinical.
01 · The clinical need
Cognitive impairment occurs across several central nervous system disease contexts, potentially including Alzheimer's disease, Parkinson's disease, major depressive disorder and other Brain Health conditions. In several neurodegenerative conditions, biological changes can precede clinical diagnosis by years, and therapeutic options remain limited.
Established research describes neurodegeneration and neuroinflammation as multifactorial processes. No single mechanism accounts for them, which is one reason additional therapeutic approaches are needed.
02 · The gut–immune–brain axis
NBX-5831 is being developed within this biological framework. Its candidate-specific mechanisms of action remain under investigation. The objective of our development program is to establish human biological evidence in patients.
The pathways described below are drawn from independent published research and are presented as scientific context. They are not presented as established mechanisms of action for NBX-5831.
Gut
The composition and functional output of the intestinal microbial ecosystem.
Immune & systemic biology
Immune and metabolic signals that may contribute to communication between the gut and the central nervous system.
Brain
Biology associated with cognition, neuroinflammation and neurodegeneration.
03 · Our approach

Scientific literature04 · Clinical strategy
NBX's planned early clinical development strategy is designed to generate human evidence by studying patients with cognitive impairment while characterizing their underlying clinical and biological profiles.
The objective is to evaluate safety, biological engagement and biomarker and clinical patterns that can guide subsequent clinical development.
Establish whether NBX-5831 engages relevant biology in patients.
Explore biological and clinical characteristics associated with observed biomarker and clinical signals.
Use human data to inform subsequent Brain Health development.
NBX-5831 remains at a preclinical stage and has not yet been evaluated in clinical trials. Its safety and efficacy in humans have not been established. Any future clinical evaluation will be subject to regulatory authorization.
05 · Human translational evidence
NBX is leveraging clinically annotated human cohorts and metagenomic data to explore the relationship between NBX-5831, cognitive phenotype, disease stage and relevant human biology.
This work is translational research. Associations observed in human cohorts do not by themselves establish causality, and they do not establish the safety or efficacy of NBX-5831.
06 · Development status
Our lead candidate, NBX-5831, is a defined oral Live Biotherapeutic candidate for Brain Health. It is progressing through preclinical development, with cognitive impairment, including MCI, as the planned clinical entry phenotype, subject to regulatory authorization and financing.

Why it matters
{{ k.why }}
{{ k.cta }}The gut microbiome is increasingly being investigated as a modulator of neuroinflammatory processes.
A 2026 review in The Lancet Neurology highlights multiple gut-to-brain pathways including immune regulation, microbial metabolites, intestinal and blood–brain barrier integrity, vagal signaling and bile-acid signaling.
Together with growing human evidence in mild cognitive impairment and Alzheimer's disease, these findings reinforce the scientific rationale for investigating precisely defined microbiome-derived therapeutics in neurological disorders.
